Tanyka Rawden, Senior Coroner for the Coroner area of South Yorkshire (West) listed in her report that: “Mr. Johnson sustained an incised wound to the neck caused by the skate of another player.”

In her report she questioned whether or not neck guards could have prevented Mr. Johnson’s death and urged the International Ice Hockey Federation (IIHF) to examine whether or not the agency should adopt such measures to prevent similar incidents in the future. “I am sufficiently concerned that deaths may occur in the future if neck guards or protectors are not worn”, she wrote.
